Thanks for the details, ensure that you deactivate and reactivate the plugin as well as performing a one time save at admin settings page as per upgrade instructions article here. Also, if you’re using any add-ons, be sure to upgrade them all as well. The add-ons will be the high likely cause here because your old Javascript files may not be able to recognize the new custom fields thus not showing them accordingly.
