Fresh WordPress 4.5 installation with the latest Super Store Finder seems to be running without issues.
If your store locator stopped working after upgrading version 4.5, it is advisable to roll back to the previous version available here http://wordpress.org/download/release-archive/ as it might cause conflict with theme/plugin javascript causing the store locator to stop working.

If you press f12 on browser console and noticed below errors, be sure to update your theme and plugins to adhere to version WP 4.5 and above.
